The Seymour School District’s nursing staff is committed to supporting the physical, emotional, and social well-being of every student. Our nursing team provides compassionate care, health education, and safe school environments, to help students stay healthy, engaged, and ready to learn.
Annual Screenings
· Vision Screening for: Grades 1, 3, 4, and 5
· Hearing Screening for: Grades 1, 3, 4, and 5
· Scoliosis Screening for: Grades 5, 7, (girls) and 8 or 9 (boys)
Student Health Requirements (Physical Exam & Immunizations)
Connecticut law requires all children who are entering Pre-K or Kindergarten to submit a current health assessment and proof of required immunizations before starting school. A physical exam must be completed within 12 months of entry using the official State of Connecticut Health Assessment Record (blue form for Kindergarten; yellow form for Pre-K). Students transferring into Seymour Schools District are also required to provide an updated physical exam, completed within 12 months. The first page of all forms must be completed by a parent/legal guardian, and the form must be signed and stamped by a licensed health care provider.

Updated State of CT Health Assessment Records along with age-appropriate vaccinations are also required for all students entering grade 7 and grade 10 (forms must be submitted prior to the start of the 7th and 10th grade year in order for students to be eligible to start school). The Oral Health Assessment portion of this form is also required for these students.
Students who enter the Seymour School District from outside the United States may be required to have a new Tuberculin Test and results reported prior to entry.

Medication Administration
A doctor’s order is required for all prescription and over-the-counter medications to be given at school. For details about the requirements of your specific school, please contact your school’s nurse.
Students with Life Threatening Allergies and Asthma
Each student with a life-threatening allergy and/or will have an Individualized Health Care Plan developed by the school nurse. Parents whose children have life-threatening allergies/asthma must provide the school with notification upon diagnosis, along with an Emergency Action Plan from the student’s healthcare provider.
